It makes my day when an application fails in beta testing with
customer in a parallel test because they say "Your not doing ...."
Looked through all the paperwork signed off and "....." is not stated,
ever. Sure this is a BIG point and how or why it was left on the
table is years in the past for this project. Today I have a bunch of
pissed off ladies who are expecting a system that will replace hrs of
human investigations (user query of existing system) when they fail to
tell us all the parts that we have to check.
This piece of business was signed off before I became an employee and
I have retro fitted 80% of all code to work in WS calls instead of
ODBC calls.
Come to find out that this was probably left off the plan because the
existing system does a piss poor job because of all the confusion
surrounding "probation" I did not realize how complex s simple word
was or is in our court system here.
Tomorrow I am going to play Mr. Missouri ans show me how you know
"Squiggly" is still on probation. Point to the current screens and
tell me how you ca read it and to what division he is assigned to.

I'm amazed at the # of shops (big time ones too) that still embrace
waterfall.

This is a complicated story for what the client wanted. If you think
of Medical records and how Electronic Medical records came in that is
close to this issue.
We are attempting to do away with that folder, the "chart".
To me that folder is to hold all of the paperwork generated by this
case, all orders of the court bla bla bla.
Well the outside of the folder has a lot of hand written data on it
and that is the start of my waterfall or "Hello Niagara where is my
barrel?"
Here in Memphis like any other city there are some people who are
repeat offenders and have a long and sorted history with the courts.
this app is an attempt to identify what the charges are, to whom
involved, what bonds were offered and if and when they were paid and
by whom. Turst me a lot more is going on but you can see is is a lot
of data.
After it is all collected I get to combine other individuals data
because crime is so much fun you have to share it with your friends.
We then determine who was the worst of the lot and see if they had
rules that put them in a specific Division (judge and courtroom) If
so apply all the peeps to the rules of the worst and that is all I
have to do. Oh yeah take 30-70 other cases and do it all at one time
for this mornings Grand Jury run.
If you can not do a waterfall for this requirement please let me know how?

The fact that you got so far along before anyone noticed that important feature X was missing was what I was reacting to. Using iterative development (agile is just one example), this would have been noticed much earlier in the process. Relying on comprehensive documentation instead of user feedback is the hallmark of waterfall.

I had user signoff a year ago on alpha and it has taken weeks of users
actually running it for them to say "hey! it's not right."

You are making one huge assumption, no matter what methodology used, many
users tend to not test until the very last minute. Often they don't realize
it until they cannot do something in production after the release. I have
seen this problem in tight iterations under agile approaches, and long
iterations like the waterfall methodology. Even with test plans. Sure they
say they test, but in reality they are busy using the old software doing the
day job, and find testing too time consuming.
Sure, all bad reasons and most of it inexcusable, but I have seen this
repeated enough over the years to know I don't assume the end users ever
test completely.
