Gary,
Tried on 2 new Compaq/HP's and also a Dell server. The only commonality really is they are using Imation floppies and didn't have any other brand to try but they were using disks frm 3 separate boxes. I know floppy backups are daft especially for OII data but they have just re-equipped and don't have a proper backup set up yet.

Alan, there used to be a problem like this many moons ago affecting certain PS/2 models which had been lobotomised for some bizarre reason and didn't automatically register a disc change. I think Ctrl-Z would force a re-init of the drive.
What make is the offending PC? Any chance you could try swapping the floppy drive from another (known to work) PC?
Best regards Gary Sutherland
